Policy Highlights

-Must be a member of USBA to purchase this insurance.

-Cost of insurance is $85 per team (15 players maximum). Coverage begins at time of purchase, and expires December 31, 2007.

-Team roster MUST be on file with USBA. If a player should be injured that is not on your roster, that player would not be covered.

-USBA Team Insurance covers both play and practice, and also includes travel directly to and from a USBA event.

-When purchasing USBA Team Insurance, the insurance fee must be sent separately, and cannot be included with a membership fee and/or a tournament entry fee.

You may pay for the USBA membership, tournament entry fee, and insurance at the same time, however, the insurance fee MUST be a separate payment. For example: Team A wants to join USBA, pay the tournament entry fee, and purchase USBA Team Insurance. The membership fee, and tournament entry fee can be purchased together, but the insurance fee would have to be a separate transaction.

Or the USBA Team Insurance can be purchased at anytime completely separate from any other kind of purchase.
