1. Print & complete the following forms.
NOTE: Only the $45, and the USBA Membership Form needs to be
sent to USBA. If you are registering for a tournament the same time you
are paying for USBA membership, the Tournament Entry Form (see #2 below)
will also need to be mailed. All other forms will need to be in your
possession and taken to each tournament. USBA officials will check all
forms prior to tournament play.

NOTE: Team memberships are non-refundable. If a
tournament cancels, you can transfer you entry fee to another USBA
tournament or request a refund, minus the $35 membership fee.
