STEPS TO REGISTER YOUR TEAM
ONLINE
1. Click here
and pay the
$35 registration fee online.
2. After paying the
registration, return to this page, and complete the following forms.
These forms DO NOT need to be sent to USBA, but be kept with you at all tournaments.
USBA tournament officials will check all forms prior to tournament play.
Forms that need to be in your possession are listed below:
*Team Roster
*Head Coach Agreement
*Parent/Player Waiver Form. Be sure to read Eligibility and Grade
Exceptions before completing the forms.
*Grade
Exception Form (if applicable)
All above forms will need to be
completed, taken to the actual tournaments, and kept with you at all times during the
tournaments. Tournament officials will check
for these.
(Girls teams DO NOT have grade
exceptions). You must have hard copies of your roster,
birth certificates &
grade exceptions to turn in to tournament officials prior to each
tournament so please make copies.
3. Purchase
"Team Insurance"

NOTE: Team memberships are non-refundable.
If a tournament cancels, you can transfer you entry fee to another USBA
tournament or request a refund, minus the $35 membership fee.
