GYM DIRECTIONS

tuesday - July 1

	Mandatory Coaches Meeting at 10:00am at North Myrtle Beach HS - 3750 Sea Mountain Hwy - Little River, SC 29566. Coaches can begin signing in at 9:00am. (If you do not attend the meeting, there will be a $20 penalty that must be paid prior to your first game - no exceptions)
	Pool play games begin at approx. 1:30

Wednesday - July 2

Pool play completed

Thursday - July 3

	Contest - 2:30PM @ North Myrtle Beach HS (3 pt & Hot Shot)
	Brackets available after contest
	Bracket play begins at 5:00pm

Friday - July 4

Bracket play & Championships

DO NOT MAIL OR FAX ANYTHING to the USBA. You will need to have ALL the documents listed below when you arrive for the coaches meeting. Please put all documents in a "manila envelope" with your team name, age group, head coaches name and contact phone # (cell or number while at Nationals) clearly marked on the outside. Once you turn this information in you will not get it back.

Each teams paperwork should be in the following order in your manila envelope:

The following forms for each player must be STAPLED AND IN THE FOLLOWING ORDER.

Parent/Player Consent/Insurance Form

Copy of Birth Certificate

ANY PLAYER THAT DOES NOT HAVE THIS PAPERWORK WILL NOT BE ABLE TO PLAY.

Admission

	Adults - $8 per day
	Students (ages 6 - 17yrs) - $7 per day (No weekly passes)
	Children 5 and under - Free
	Each team will receive 3 wristbands for coaches and scorekeepers regardless of the number of coaches you have. They must be worn all week or you will have to pay admission.
