mONDAY - June 23

	10:00am-12:00am: Team Check In at North Myrtle Beach HS - 3750 Sea Mountain Hwy - Little River, SC 29566
	1:00pm-1:45pm: Mandatory Coaches Meeting at North Myrtle Beach HS (If you do not attend the meeting, there will be a $20 penalty that must be paid prior to your first game - no exceptions)
	Pool play games begin in the afternoon

	Brackets will be announced after contests. Please do not call for bracket play times as we will not return those calls. You must come to North Myrtle Beach HS to see where and when you play.

We WILL NOT accept request for certain game times. Once the schedule is complete it will NOT be changed. Teams must be prepared to play following the Mandatory Coaches Meeting.

TEAMS ARE RESPONSIBLE FOR THEIR OWN MEDICAL EQUIPMENT - ATHLETIC TAPE, PRE-WRAP, BAND-AIDS ETC. THE USBA WILL HAVE ICE AVAILABLE AT EACH GYM.

DO NOT MAIL OR FAX ANYTHING to the USBA. You will need to have ALL the documents listed below when you arrive for the coaches meeting. Please put all documents in a "manila envelope" with your team name, age group, head coaches name and contact phone # (cell or number while at Nationals) clearly marked on the outside. Once you turn this information in you will not get it back.

Each teams paperwork should be in the following order in your manila envelope:

The following forms for each player must be STAPLED AND IN THE FOLLOWING ORDER.

Parent/Player Consent/Insurance Form

Birth Certificate

Grade Exception Form AND Current Report Card Indicating Grade (only grade exception players need this)

ANY PLAYER THAT DOES NOT HAVE THIS PAPERWORK WILL NOT BE ABLE TO PLAY.

(Ages 9-14 can have up to three grade exceptions / Ages 15 & up can have unlimited grade exceptions)

IF YOU DO NOT HAVE THE ABOVE INFO (a-f) WHEN ARRIVING FOR THE COACHES MEETING YOU WILL NOT BE ALLOWED TO PLAY. All info will be turned in when you arrive. DO NOT mail or fax anything. Be sure to make copies as documents will not be returned.

Admission

	Adults - $8 per day
	Students (ages 6 - 17yrs) - $7 per day (No weekly passes)
	Children 5 and under - Free
	Each team will receive 3 wristbands for coaches and scorekeepers regardless of the number of coaches you have. They must be worn all week or you will have to pay admission.
