Contest at 6:00pm @ North Myrtle Beach HS - brackets will be announced at the conclusion of the contests. |

Coaches Meeting after or during contest (approx 7:30pm) - You must be present as we will NOT return phone calls letting you know when and where your first bracket game will be. You must attend the meeting to get your game time. |

You may register for contests between 12:00 noon and 5:00pm. Two players per contests, per team. |

DO NOT MAIL OR FAX ANYTHING to the USBA. You will need to have ALL the documents listed below when you arrive for the coaches meeting. Please put all documents in a "manila envelope" with your team name, age group, head coaches name and contact phone # (cell or number while at Nationals) clearly marked on the outside. Once you turn this information in you will not get it back.
Each teams paperwork should be in the following order in your manila envelope:
1. Team Roster with Jersey Numbers
The following forms for each player must be STAPLED AND IN THE FOLLOWING ORDER.
3. Birth Certificate (clear copy)
4. Parent/Player Consent/Insurance Form
5. Grade Exception Form AND copy of a Current Report Card Indicating Grade (only grade exception players need this)
ANY PLAYER THAT DOES NOT HAVE THIS PAPERWORK WILL NOT BE ABLE TO PLAY.
(Ages 9-14 can have up to three grade exceptions)
TEAMS ARE RESPONSIBLE FOR THEIR OWN MEDICAL EQUIPMENT - ATHLETIC TAPE, PRE-WRAP, BAND-AIDS ETC.

USBA reserves the right to revoke any teams membership and to deny teams entry to any sanctioned USBA event.

The deadline for all USBA regular season tournaments is the Saturday prior to the event. No exceptions.
